Birth Certificates
The Health Department can issue birth certificates for the years 1930 to present, within the State of Florida. Birth certificates for persons born in the State of Florida in the years prior to 1930 must be obtained from the State Office of Vital Statistics in Jacksonville, Florida.

Birth Certificates are Needed For:

  • Admission to public school
  • Obtaining a driver?s license
  • A work permit
  • The right to vote
  • Eligibility for retirement
  • Social Security benefits
  • Proving citizenship
  • Obtaining public assistance for dependant children

Birth Certificates Can be Issued to:

  • Parents
  • Individual 18 years of age or older, with proper identification
  • Legal guardians

Fees:

  • Citrus County - $12.00 for the first copy, $8.00 for additional copies for the same person

Include the following information:

  • A photocopy of your driver's license for identification must be included
  • Name of Child
  • Date of birth and county of birth
  • Mother?s maiden name and father?s name
  • Relationship to the person on the birth certificate

Death Certificates
The Citrus County Health Department issues death certificates for the years 2006 to the present for those individuals who have passed away in Citrus County. Death Certificates for other years or counties must be requested from Jacksonville.
